After processing and converting to grayscale in Camera Raw, I opened the file in Photoshop, smoothed the skin with the Spot Healing brush and the Clone tool. Finally I added an oval burn to darken the edges. Very importantly, I carefully sharpened the final image in Photosop before uploading it to JPG.
