Yes, real. I taught Anatomy and Physiology labs at Indiana University for 12 years. I always loved teaching skeletal. I asked the department head why the specimens were so small. He told me most come from India. Don't know if that's true, but curious. The details (such as sutures), light, and tone are amazing. Love, love, love this image.
