Here is how it was done:
I was going back and forth with the focus ring while having shutter open to get the 'smearing' effects of the headlights and the overall soft focus effect.
One thing you should keep in mind: Camera has to be stranded very tight on the tripod so it won't shake at all, otherwise you going to lose sharpness.
